Registration of authorisations and legal representation within the EESZT

From now on, after registration of appropriate representation rights it will be possible to have other people’s ePrescriptions filled without submission of the prescription certificate and to access their health data on the platform of the EESZT Public Portal.

 

I have a child under the age of 14

 

After providing proof of personal identity, through the Government Customer Service the legal representative can submit the application for registration of legal representation. Presentation of the document proving legal representation rights is necessary for this purpose. 

For the procedure in the Government Customer Service it is not necessary for both parents to appear, as they may give authorisationto each other for the procedure. Therefore, the parent present may request registration of his/her own right of representation and of the other party as an authorised person by presentation of the appropriate instruments proving authorisation.

NECESSARY DOCUMENTS AND INSTRUMENTS:

  • Application for registration of legal right of representation within the EESZT
  • In relation to spouses, the birth certificate of the child is necessary for proving authorisation.
  • In case of a lawsuit filed for exercising custody or for divorce, the court judgment handed down for exercising custody is to be attached.
  • If the parties entered into an agreement for resolving the exercise of custody, it is necessary to present the approval decision of the court. 
  • The court decision restoring parental right of custody needs to be attached if the right of custody revoked earlier is restored.


 

I am the legal representative of a partly incapacitated minor aged 14-18 in custody or under guardianship

A third party—authorised for the procedure by the legal representative—may also apply for registration of the right of legal representation.

  • Decision of the child protection authority on appointment of a guardian.


 

I represent an incapacitated or partly incapacitated adult as a custodian 

 

After providing proof of personal identity, through the Government Customer Service the legal representative can submit the application for registration of legal representation. Presentation of the document proving legal representation rights is necessary for this purpose. 

A third party—authorised for the procedure by the legal representative—may also apply for registration of the right of legal representation.

The third party authorised for the registration procedure in the Government Customer Service needs to present the following documents: 

 

I represent a different natural person or 
I wish to register an authorisation on behalf of an old person with reduced mobility

 

The represented person, i.e. the authorising person, may grant authorisation to you by filling in the electronic form on the New authorisation tab under the Patient consent menu item of the Public Portal .

If the authorising person is unable to appear in person at the Government Customer Service, in place of him/her the authorised person may also request registration of the right of representation by submission of the filled in and signed (2 pages each!) documents downloadable through the following links:

How can I have another person’s ePrescription filled without paper? 

 

1. Upon your request your right of representation (authorisation or legal representation) is registered within the EESZT system. 

2. For filling an ePrescription, it is sufficient to provide the TAJ number of the patient at the pharmacy.

3. Based on your personal data provided earlier, the pharmacist will identify you within the EESZT system and provide you the product prescribed for the patient.


If you have the ePrescription of another person filled without a prescription certificate, in each case please take with you a document suitable for establishing personal identity (personal ID card, driving licence or passport and address card), on the basis of which the pharmacist identifies you!

 

 

How can I access the health data of the person I represent? 

 

If you hold the right of representation registered within the EESZT, through the Public Portal you can access certain health data of the represented person stored within the EESZT and may also issue a statement on digital patient consent on his/her behalf. You may do so after selecting the TAJ number of the represented person after login. 

 

 

Can I simultaneously represent more than one person?

 

Yes. Any number of rights of representation may be registered in the system, thus if you wish to have the ePrescriptions of several persons filled, or to access their health data, this is also possible. 

 

 

It is possible to revoke authorisations given earlier on the Public Portal of the EESZT or by personal appearance at the Government Customer Service. 

 

If you do not set the starting and ending date of authorisation, the given authorisation is valid from the date of registration until revoked. You may revoke the individual authorisations with the following downloadable declarations:
